Car not coming up to proper temperature

A couple of quick questions:

1) Has anyone touched the cooling system?

2) How long are you driving (not idling) the engine when you observe the low temperature?

3) Are you running the HVAC system fan at speeds three or higher?

4) Have you noticed the radiator fan on?
